QLIMG wins recognition award

The Queens-Long Island Medical Group (QLIMG) recently won an award from the National Committee for Quality Assurance (NCQA).
QLIMG became the first group in the state to receive Level 3 recognition by the NCQA, through its Physician Practice Connections - Patient-Centered Medical Home award.
The Committee’s Physician Recognition Program spotlights physicians who provide high quality care to their physicians.
Nicholas Vogiatzis, MD, who operates from the QLIMG Richmond Hill Medical Office, spoke about this merit.
“Needless to say, we are all extremely proud of this achievement, not only in terms of what it means to us, but what it means to our patients - that they can be confident in the quality of health care they receive at our QLIMG Medical Offices,” Vogiatzis said.
The patient-centered medical home is a model for care that seeks to strengthen the physician-patient relationship through coordinated care and a long-term healing process.
A physician-led team is responsible for providing all the patient’s health care needs and arranges for appropriate care with other physicians if necessary.
QLIMG received this honor based on their evaluations of such functions like Patient Self-Management Support; Advanced Electronic Communications; Referral Tracking; Care Management; and Performance Reporting and Improvement.
Vogiatzis said that “QLIMG is committed to ensuring that we continue to strive towards excellence in patient care by investing in quality of care and process improvement initiatives.”
