Application requirements are as follows:
Senior Citizens (65+) – Proper proof of age, such as Medicare card, and a valid photo ID that includes birthdate and signature.
Medicare Recipients – Copy of Medicare card and a valid photo ID that includes birthdate and signature.
Disability/SSI Applicants – Physician’s/certifier’s diagnosis (including duration of disability), licensing number, and signature, and valid applicant photo ID that includes birthdate and signature. SSI applicants applying under Serious Mental Illness criteria must provide their Social Security number for verification purposes.
