Senior Source program helps older adults

Through Selfhelp Community Services, Inc.’s program, Senior Source, older adults can get guidance to assist with insurance, Medicare and end of life planning.

Kaplan. “Our Senior Source staff is uniquely qualified to guide the client and their family through the difficult decision-making process that often accompanies this stage of life, to ensure that our clients are receiving the expert resources that they need - whether they be medical, legal, financial, or social - to help guarantee their best quality of life.”
A comprehensive care management program, Senior Source assists seniors and their families by “addressing immediate needs or creating a personalized plan of care for the future.” One way in which care managers do this is through providing guidance as it relates to health care options, insurance and Medicare and Medicaid.
When clients first start the Senior Source program, their physical, emotional and social health is assessed by care managers, who centralize important information as it relates to the client so that they can monitor things while also coordinating care. Throughout the process, the client’s family and physicians are kept informed.
Through its access to assistive technology, case managers are also able to keep “bridge the gap between consumer, distant family members, and health care providers.”
Senior Source offers various fee-based services that clients and their family members can chose from. Among them are telephone support and arranging and managing home care visits.
